Community Care of Western New York Honors Dan Horn Pharmacy with 2025 Founder’s Award

ALLEGANY - Community Care of Western New York (dba HomeCare & Hospice) has named Dan Horn Pharmacy of Olean a recipient of its 2025 Founder’s Award, the organization’s highest honor. The award was presented during the annual board luncheon in July, celebrating exemplary community partners who embody the mission and values of home health and hospice care.



Founded in 1986, the Founder’s Award is given to individuals or organizations that have made significant contributions to the care and dignity of patients and participants across the four-county service area. Dan Horn Pharmacy was honored for its long-standing collaboration, notably with Total Senior Care, and its consistent commitment to timely service, community health, and compassionate partnerships.

Josh Horn accepted the award on behalf of the pharmacy team, sharing reflections on what community partnerships like this one mean to small businesses.

“Local businesses need to support organizations like HomeCare & Hospice and Total Senior Care because the work they do has a real, fundamental impact on people’s quality of life,” said Horn. “It’s not just beneficial to the community - it’s mutually beneficial. We’re honored to have been part of this mission since the beginning, and it’s been a privilege to grow that partnership over the past 16 years.”

From fast turnaround times on prescriptions to organizing onsite vaccine clinics for senior care participants and staff, Dan Horn Pharmacy has played a key role in ensuring continuity of care for some of the most vulnerable members of the community.

“We’re proud of the speed and reliability of our service - it matters,” Horn added. “We believe in being responsive, present, and personal, and we’re grateful to be recognized for doing what we love in support of our neighbors.”

Melissa Sullivan, Chief Executive Officer of Community Care of Western New York, praised the pharmacy’s ongoing dedication and community-first mindset. “Dan Horn Pharmacy exemplifies what it means to be a true partner in care,” said Sullivan. “They bring reliability, flexibility, and heart to every interaction - whether it’s filling an urgent prescription, organizing a vaccination clinic, or simply showing up with a willingness to help. Their work has enhanced our ability to serve, and we’re proud to honor them with this award.”

Sinclair Pharmacy of Warsaw was also honored with a 2025 Founder’s Award during the event. Additionally, several board members were recognized for milestone years of service including Dr. Pierre Dionne, Community Care of Western New York Board - 35 years; Maureen Mooney-Myers, HomeCare & Hospice Foundation Board - 20 years; and Edward Leising, HomeCare & Hospice Foundation Board - 15 years.
